چکیده

 Continuous glucose monitoring for type 1 diabetes mellitus will go forward for refinement as a systematic review. The scope of this topic, including populations, interventions, comparators, and outcomes, will be further developed in the refinement phase.  When key questions have been drafted, they will be posted on the AHRQ Web site and open for public comment. To sign up for notification when this and other Effective Health Care (EHC) Program topics are posted for public comment, please go to Nomination Summary: The nominator is interested in the comparative effectiveness of various treatment strategies with and without continuous glucose monitoring for type 1 diabetes on HbA1c values. The nominator suggests that many patients benefit from continuous glucose monitoring with daily insulin injections or insulin pump therapy; however, insurance coverage is limited to private insurance companies. The nominator asserts that variations in insulin needs can be closely monitored and recorded with continuous glucose monitoring, which may lead to improved insulin dosing and better glucose control. Staff-Generated PICO: Population(s): Adults and children with type 1 diabetes mellitus Intervention(s): Multiple daily injections with continuous glucose monitoring or insulin pump therapy with continuous glucose monitoring Comparator(s): Multiple daily injections alone or Insulin pump therapy alone Outcome(s): HbA1c levels Key Questions from Nominator: 1. What are the comparative differences in HbA1c levels when comparing various treatment options including multiple daily injections, insulin pump therapy, daily injections with continuous glucose monitoring, and insulin pump therapy with continuous glucose monitoring?

Design and rationale of the IN CONTROL trial: the effects of real-time continuous glucose monitoring on glycemia and quality of life in patients with type 1 diabetes mellitus and impaired awareness of hypoglycemia

BACKGROUND Hypoglycemia is the main side effect of intensified insulin therapy in type 1 diabetes and recognized as a limitation in achieving glycemic targets. Patients with impaired awareness of hypoglycemia have a threefold to sixfold increased risk of severe hypoglycemia. Real-time continuous glucose monitoring may help patients with type 1 diabetes to achieve better glycemic control with le...

Combined insulin pump therapy with real-time continuous glucose monitoring significantly improves glycemic control compared to multiple daily injection therapy in pump naïve patients with type 1 diabetes; single center pilot study experience.

OBJECTIVES This study assessed the safety and clinical effectiveness of the training protocol for initiating insulin pump therapy with real-time continuous glucose monitoring (MiniMed Paradigm REAL-Time System) in a stepwise approach on pump naive subjects with type 1 diabetes compared to a control group who remained on multiple daily injection (MDI) therapy. METHODS This was a 15-week treat-...

Professional continuous glucose monitoring in subjects with type 1 diabetes: retrospective hypoglycemia detection.

BACKGROUND An important task in diabetes management is detection of hypoglycemia. Professional continuous glucose monitoring (CGM), which produces a glucose reading every 5 min, is a powerful tool for retrospective identification of unrecognized hypoglycemia. Unfortunately, CGM devices tend to be inaccurate, especially in the hypoglycemic range, which limits their applicability for hypoglycemia...

Real-Time Continuous Glucose Monitoring Significantly Reduces Severe Hypoglycemia in Hypoglycemia-Unaware Patients With Type 1 Diabetes

OBJECTIVE To evaluate the effect of continuous glucose monitoring (CGM) on the frequency of severe hypoglycemia (SH) in patients with established hypoglycemia unawareness. RESEARCH DESIGN AND METHODS We conducted a retrospective audit of 35 patients with type 1 diabetes and problematic hypoglycemia unawareness, despite optimized medical therapy (continuous subcutaneous insulin infusion/multip...

Insight into hypoglycemia in pediatric type 1 diabetes mellitus

Hypoglycemia is a common complication of insulin treatment in type 1 diabetes mellitus and can occur in any patient with diabetes when glucose consumption exceeds supply. Many studies have been done to elucidate those factors that predict severe hypoglycemia: younger age, longer duration of diabetes, lower HgbA1c, higher insulin dose, lower Body Mass Index, male gender, Caucasian race, underins...
